A payer of contributions, meaning the employer makes an application. Employer should report the employee up to ZUS within 7 days from the date of signing the work contract (to this purpose one should fill in the ZUS ZUA form). Moreover the employer establishes the amount of contributions, collects, accounts and transfers the contributions to ZUS.

The following data is needed for reporting and accounting insured person: PESEL, NIP [taxpayer's identification number] or the passport number and series if the foreigner does not have one of these documents/numbers.
For every newly admitted employee who does not possess the insurance card, the institution that entrust work is obliged to issue it immediately after employing the worker. In this case at the moment of employing the worker institution provides the employee with the application form. An application must be stored in employee’s personal files.
The employer can also issue a certificate to the employee certifying the fact that he is liable to the health insurance.
