Types of decision and documents in visa obtainment procedure

When applying for a visa, one is supposed to gather all the necessary documents adequate for the aims of the planned entry. Polish legislation provides 21 anticipated purposes of acquiring a visa. These are the following instances:

“01” – a visa is issued for the tourist purpose;
“02” – a visa is issued for the visit purpose;
“03” – a visa is issued for the sport event purpose;
“04” – a visa is issued for the purpose of conducting a business activity;
“05” – a visa is issued for the purpose of conducting a cultural activity or participating in an international conferences;
“06” – a visa is issued for the purpose of conducting an official business by the foreign government representative and international organizations;
“07” – a visa is issued for the purpose of performing a job dependent on the seasonal rhythms;
“08” – a visa is issued for the purpose of performing a job;
“09” – a visa is issued for the purpose of conducting undergraduate studies, graduate studies and doctoral studies;
“10” – a visa is issued for scientific, training, didactic or other than the purpose defined  in article 28 para.1 clause 8 of the Act
„11” – a visa is issued for the purpose of using a temporary protection;
„12” – a visa is issued for the humanitarian purpose, on the grounds of the country interest or an international commitment’
“13” – a visa is issued for the purpose of joining the citizen of the Member State of European Union; the Member State of the European Free Trade Association (EFTA) – the agreement side of the European Economic Area or Swiss Confederation or staying with them
„14” – a visa is issued for the purpose of participating in the cultural or educational exchange program or humanitarian aid program or holiday job program for students (instances of the program are regulated by the international agreement, when the Republic of Poland is one of the sides, there is a name of the program on a visa sticker);
“15” – a visa is issued for the purpose that is different than those defined according to article 28 para.1 clause 1-13 and para. 2 and 3 of the Act.
“16” – a visa is issued for the purpose of participating in the proceedings of granting the asylum
„17” – a visa is issued for the purpose that is defined in article 43 para.4, article 61 para.3 or article 71a para.3 of the Act;
„18” – a visa is issued for the purpose of resettlement; as the member of the family of the repatriate;
“19” – a visa is issued for the purpose of realization of permit for temporal residence; permit for the settlement or permit for long-term stay of the resident of the European Community;
„20” – a visa is issued for the purpose of using the authorization resulting from possessing the Polish Charter;
“21” – a visa is issued for the purpose of repatriation.

The purpose of issuing a visa, in the case of points 1 to 15, is not dependent on the type of a visa, whether national or harmonized visa. The purposes of issuing a visa, marked with numbers 16 to 21 – relate to national visas only. It should be remembered that each decision on issuing a visa is reached by the consul of the Republic of Poland, and it is him who is allowed to deny a visa as well. These are the basic documents requisite for applying for a visa:

  • A foreign passport (valid for at least 3 months from the date of the planned exit from the territory of Republic of Poland) and an Identity Document (in some countries such as the Republic of Belarus it is one and the same document).

ATTENTION! In case of possessing 2 or more passports, one is obliged to submit all the documents in the office.

  • copy of the personalized page of the entry document (a page with photograph)
  • 2 photographs, size 35 mm wide x 45 mm high. Photographs must fulfill the following criteria:
  • Photos should be in color, sharp, clean and high-contrast
  • Photos should be printed on a high quality paper and taken against light background
  • Photos should not be older than 6 months and should enable to indentify the applicant person
  • Photos should be taken directly opposite, show eyes visibly, the face from both sides, from the crown of the head till the upward part of the shoulders, in order to cover 70-80% of the photo
  • Photos should present a person without a headgear, looking straight, with his mouth closed
  • Photos should not be taken so that one’s hair covers the face

If a person wears glasses, a photograph should fulfill the following criteria:

  • Lenses should not be tinted
  • The frame should not cover any part of the eye
  • There should not be any reflection of light

In case of a person wearing religious headgears, a photograph should fulfill additional criteria:
-   A beard, a forehead and both cheeks should be clearly visible.
-   There should not be any shadows on the face, caused by a headgear.

  • Insurance Policy on minimum 30.000 EUR, including the time of stay indicated in the visa form, valid on Schengen territory (in case of temporary visa) or documents which verify the possession of health insurance as defined by the medical benefits regulation financed form public funds or  covered by insurance company, the costs of treatment on the territory of Republic of Poland (in case of national visa).
  • Visa Application Form filled in correctly (answer on each point written with capital letters, the name and the surname given in Latin alphabet, as in passport)
