What is an address registration?
A person staying in particular place in Poland, at the same address longer than 3 days, has to meet address registration obligation consisting in registering at address of a permanent or temporal residence. A person leaving a permanent or temporal residence place is obliged to unregister from that address.
Issues related with address registration are regulated by the Law of 10 April 1974 on population registers and identity cards (Journal of Laws 2006, No. 139, item. 993, as amended.)
The address registration obligation comes into force up to the end of the fourth day from the moment of crossing Polish border. If you are staying in a hotel, hostel, sanatorium or similar place related with work, studying, treatment or resting, registration obligation is in force from the moment of arrival.
Procedures related with address registration can be settled in a community or city hall (in a citizen affairs department) adequate to your place of residence.
Foreigners temporarily staying in hotels, facilities giving accommodation due to treatment or rest, have to register at appropriate point of such facility.
Registration procedures are settled free of charge and straight away (at the moment of acceptance of your application). As it is a material-technical procedure, there is no option of revocation.
In exceptional cases when registration or unregistration is necessary in a form of administrative decision, a payment may be required. In such case, administrational procedure is executed which is completed with an administrative decision. In such a case, a revocation is possible.

Polish law does not limit number of people that can be registered in a single dwelling.
If a particular type of formalities requires submitting a form, you can acquire such form in a particular office, its website.
NOTE!
Forms are not adjusted to current legal status. Formerly, temporal stay registration could be made for a period shorter or longer than 2 months. At present, it is a period of 3 months. For example, if you see a form „Zgłoszenie pobytu czasowego trwającego ponad 2 miesiące” (application for a temporal stay for a period longer than 2 months) it is suitable for registration of a stay for a period longer than 3 months.

Foreigners having registered address for a permanent or temporal stay for a period longer than 3 months, are given a identity number PESEL

Address registration on the basis of an authority certified by a notary
When an owner of a dwelling issues an authority certified by a notary indicating a lessee as a person authorized for registering at given address or to register other persons and makes a property act available for the time of registration procedures, the authorized person, e.g. the lessee, has the right to make registration in a community office without the presence of the dwelling owner. |
What is the PESEL number?The number in the General Electronic System of Population Record (pol. Powszechny Elektroniczny System Ewidencji Ludności; PESEL), that contains data of people permanently residing in Poland or people obliged to have this number due to other requirements, is conferred by the Minister of Internal Affairs and Administration. The PESEL number is given in a community office of your registration address. The PESEL number is conferred to foreigners registered in Poland for a temporal or permanent residence longer than 3 months and those who are subject to Polish social or health insurance. |
