Required documents:
- Completed and signed submit form of permanent stay registration
- confirmation of unregistering from the previous permanent stay address – in case when a person was previously registered in Poland for a permanent stay
- a foreigner presents a passport or other document enabling identity determination and a document confirming legality of the stay in Poland, e.g. a visa, a residence permit etc. (please check in your office what document should you present)
additionally:
- for verification, original document confirming legal rights to a dwelling: e.g. a rental contract, a legal decision, an administrative act, an abstract from a land register or other document confirming legal rights to a dwelling.
NOTE!
When completing a form, you need to have a confirmation of stay in the dwelling where you are going to be registered. The confirmation is made by the person having legal rights to the dwelling (e.g. its owner). The most often situation is that offices require the presence of such person.
When all procedures are finished, a person submitting application receives a document confirming registration (valid for 3 months period).
