2 types of certificates are used: certificates for transport of animals of non-trade character (according to the Commission Decision no 2004/824/WE from 1.12.2004 establishing model of a health certificate for non-trade transport of dogs, cats and ferrets from third countries to EU) and of trade character (according to the Commission Decision no 2004/595/WE from 29.07.2004 regarding establishing model of a health certificate for transport of dogs, cats and ferrets from third to EU for trading aims).
VETERINARY CERTIFICATE FOR DOMESTIC DOGS, CATS AND FERRETS ENTERING EU USED FOR NON-TRADING AIMS
First four blank spaces have to be completed: owner/person responsible for pet, description, identification, inoculation against rabies. An application should be completed in Polish or English.
In order to be eligible to be transported within the EU and outside the EU, a pet should be examined by a vet 24 hours before leaving and there should be certification of inoculation against rabies put in a passport.

IMPORTANT! Information put in a passport and health questionnaire should be readable, with capital letters, in Polish and language of a country of origin. A vet completing forms should use official names of travel countries. |
