In different countries, different animals have the right to receive passports. Passports are issued by vets in their countries. Information about rules of issuing passports for animals can be obtained e.g. in an Agricultural Ministry and about rules of animals borders crossing at Border Guard.
Entry rules in the EU depend on the country of origin and different sets of documents are in force:

Titration test result is not required provided that an animal does not travel by road or railway transit through a third country or a territory other than included in part C of the annex II to the Commission Regulation (WE) no 1467/2006 from 04.10.2006. Legal basis:
A certificate concerning health of transported animals, issued by a vet, is necessary.
