A copy from the National Court Register (KRS, Krajowy Rejestr Sądowy), business record or other appropriate record evidence (that includes current data) - when the entity entrusting the work conducts economic activity;

a) within 3 years preceding the application for a work permit graduated from a school or a college in Poland, or in another EEA state or the Swiss Confederation, or

A document from the appropriate register confirming the legal status and the form or nature of activities carried out by a foreign employer (a foreign counterpart of the KRS copy)

A letter of delegation (issued by a foreign employer indicating the name of the posted employee, the name of the post and place of work in Poland)

A copy of a long-term cooperation contract between Polish  and a foreign employer

Copies of the contract under which service in Poland is provided or these sections of the contract which specify the obligations of a foreign employer (in such cases when  the employer does not want to disclose other terms of the contract, and they are insignificant in terms of procedures related to  issuing a work permit) or, in the case when the contract has not  been concluded yet, a statement made by a foreign employer and a copy of the relevant portion of the contract , a foreign employer shall  provide immediately after its conclusion, the documents may be made in a standard written form
