How to fill in the Visa Application Form?

Visa Application Form should be filled in according to the following criteria:

  • it should be filled in with Latin letters on a computer, typewriter or with writing imitating the print, without deletion and correction, in accordance with appropriate entry contained in entry documents
  • in the application form all points should be filled in
  • if some information does not concern the applicant, put the mark “-“
  • in the left upper corner there should be a photograph sized 3,5 x 4,5 cm.

ATTENTION! The forms which do not fulfill above requirements are usually denied.

Instructions on how to fill in the visa application form step by step (numbers correspond to symbols of individual blanks in the visa application form)

Visa Application Form, in each individual points, should be filled in as follows:

1. The notation must be exactly the same as in your passport
2. Filled in by the people who used to have a different surname. Those who did not have a different surname should put the mark “-“
3. The notation must be the same as in your passport (Latin transcript)
4. Write the date of birth in the following order: year, month, date.
5. Write if you possess one. If a person does not possess it, put a mark “-“
6. Write the current name of place and the country of origin
7. Write a current nationality
8. Write a nationality one got when he was born
9. Put a cross in the relevant box
10. Put a cross in the relevant box, according to the actual state
11. Write your father’s name
12. Write your mother’s name
13. Put a cross in the relevant box, with accordance to the document submitted in the office.
14-18 Put a cross in the relevant box.
19. Write the number of days you would like to stay on the territory of the Republic of Poland during the validity of visa
20. If, during last three years, a person submitting a form had Polish visas, he is obliged to give the numbers and the expiry dates of each visa. If he did not have one, write “none”, “non-applicable” or “n/a”.
21. Write in case of transit, if it does not concern you, put a mark “-“.
22-24. Put a cross in the relevant box.
25. Write the number of days you would like to stay on the territory of the Republic of Poland during the validity period of visa
26. If, during last three years, a person submitting a form had Polish visas, he is obliged to give the numbers and the expiry dates of each visa. If he did not have one, write “none”, “non-applicable” or “n/a”.
27. Write in case of transit, if it does not concern you, put a mark “-“.
28. Write the names of the countries which one has gone to during last 5 years. If it does not concern you, put a mark “-“.
29. Cross an appropriate square (one can only cross one travel destination). In case of choosing the “other” option, one is obliged to specify his/her reasons for the planned travel.   
30. Write the date of the planned entry to the territory of RP (first stay, in case of applying for a multiple visa).
31. Write the date of a planned leave from the RP territory (the last leave, in case of applying for a multiple visa)
32. Write the name of the border crossing through which the entry to the RP territory will take place (first – in case of applying for a multiple visa)
33.Write the kind of the vehicle in which the entry to RP will take place
34.Write the data of the party accepting the foreigner in RP.
35-36. Cross an appropriate square and detail the information.
37-42. Fill in, if the family members appearing in the applying passport, are travelling with you. It their personal data appears in the passport but they are not accompanying you in your travel,  you should write "-"  in the space provided.
43. It is filled by a foreigner who is applying for a visa to the commanding officer of the border control point. If it does not concern you, write "-" in the space provided.
45. Write the address of the applicant (in the native country)
46. Write your telephone number
47. Write the name of your place of residence and the date of your submitting of the application.
48. You need to sign the paper.

ATTENTION!
In order to verify the data from the visa questionnaire, a consul may demand that an applicant should deliver additional documents. It can be, for example:
- employment confirmation or business activity document,
- confirmation of one’s registration place,
- confirmation of one’s financial resources,
All the documents must be submitted in their original version, together with their copies attached, and must be submitted individually.
